This agreement outlines the terms, conditions, and responsibilities of users of the Community Toolbox. This is a project organised by the Internet of Caring Things® (IoCT™) team from the National Innovation Centre for Ageing (NICA) supported by Newcastle University and funded by the North East Combined Authority. The Community Toolbox aims to provide free technology to the Newcastle community to make data collection more accessible.
As the borrower, I agree to borrow the selected device(s) for the specified duration approved by the team. Currently available devices in Community Toolbox: Telraam, Fitness Tracker, Pet Sensor, Emotiv MN8
Training
I acknowledge that I received training on properly using and maintaining the borrowed device. Additionally, the training included a verbal description of the equipment parts, an explanation of documents (checklist, user guide, data protection documents and feedback form) inside the toolbox, and a description of how and where the IoCT™ team will contact me for tool setup purposes and weekly check-ups.
Responsibilities
As a participant in the Community toolbox who is borrowing technology and equipment, I acknowledge and agree to the following responsibilities:
Safe and Proper Use: I will use the borrowed technology and equipment safely and responsibly. I will follow all training guidelines and safety instructions provided during the training session.
Maintenance and Care: I am responsible for the security and safety of the borrowed device while it is in my custody. This includes safeguarding the equipment from damage, loss, or theft. In the event of damage or malfunction, I will promptly notify the IoCT™ team through the Voice platform or directly to NICA’s phone number: 01912082503
Return of Equipment: I will return the borrowed device in the same condition as it was received, on or before the agreed-upon return date specified in this agreement.
Liability: I understand that I may be held liable for any damage, loss, or theft of the borrowed device that occurs during my possession. I understand that it is my responsibility to ensure the supervision of the device if close to children.
* Disclaimer: Newcastle University is not responsible for any physical or personal damage to my belongings caused by the borrowed device while it is in my possession.
I understand that my data is collected and processed to understand who is borrowing the community toolbox device and where it is being located, the data will be protected by applicable data protection laws. The IoCT™ team will need a form of ID to make sure, the device is sent to the correct person and address.
I understand that any identifiable data will be eliminated from the system as soon as the toolbox is returned. The non-identifiable data collected by the tool being borrowed will be stored for a minimum of 3 years by the IoCT™ team for project evaluation, research, and compliance purposes.
I consent to collecting and processing my data for the stated purposes.

Once the device is returned, the IoCT™ team will check that all the device elements are complete and in good condition. After this process is finalised, the program will provide me with a report summarising the collected data. This report will include the following:
Data Summary: An overview of the data collected during the borrowing period, including any significant findings or trends.
Privacy Protection: A statement ensuring that any personal or sensitive information collected will be anonymized or de-identified to protect the privacy of individuals.
Data Usage: A description of how the collected data will be utilized, including any research, analysis, or project-related purposes.
By agreeing to Community Toolbox Loan Agreement, I acknowledge that I have read and understood the terms and conditions outlined in this agreement. I additionally agree to adhere to safety guidelines during the device usage.
